Print one record per service event. No input fields; no data is sent or stored. Use portrait orientation and disable browser headers/footers.

Never open a pressurized filter. A zero gauge reading alone is not permission to open it. Use accessible records or a qualified professional's report. Do not operate unsafe equipment to complete this sheet. Not an opening, chemical-cleaning or restart procedure.

1. Match the equipment and evidence

Write “Unknown” for missing evidence. Do not invent a new-element baseline.
Record fieldEvidence / answer / attached record
Tank full model / revision; element identifier
Applicable equipment and element manual / revision / pages
Service date / qualified provider / approved method reference
Original NEW-element pressure / date / RPM / system conditions
Gauge reliability / confirmed by whom
BEFORE service: pressure / RPM / observed flow / date
AFTER service: pressure / RPM / observed flow / date
Comparable system conditions? Differences / debris or algae events
Media / core / end-cap condition (safe inspection or report)
Band-only observations, recorded separately
Prior service dates / comparable intervals / changed conditions
Exact-manual replacement criterion AND timing / professional assessment

2. Diagnose before purchase

Decision: service review / system or gauge diagnosis / confirmed replacement / unresolved

Missing evidence / responsible provider / follow-up: __________________________________

If replacement supported: confirmed SKU / compatibility / quantity: _____________________

No universal pressure or age cutoff. A cleaning trigger is not automatically a replacement verdict. Compare readings at comparable RPM and system conditions; do not adjust equipment merely to fill this sheet. Keep the original new-element baseline distinct from an older element's after-cleaning reading. A single tank gauge is not a direct differential-pressure measurement.

Assess the whole element. Unicel says broken or missing bands alone need not require replacement, while describing loose bands as a possible wear indication. Record torn media, core and end-cap damage separately. One shortened service interval is not proof of expiry. New media does not repair unsafe equipment or certify swimming safety. Hold unresolved purchases.
